Community Health Worker (CHW)/Case Manager - Full time to provide torture survivors and their families supported referrals and coordination for housing, shelter, employment, assistance in obtaining health care and insurance enrollment, education and support with immigration issues, and referrals to substance abuse and mental health services. Conduct outreach in the Cambodian community, educational home visits, provide outreach education workshops, design and distribute health promotion materials in own language and English. Collaboration with community agencies, coordinate resources/referrals and network with Community leaders. Experience in community outreach and human services or related education. Understand mental health, cultural beliefs, practices and culture. Bilingual skills in English and Khmer.
