African Advisory Board - The African Advisory Board was formed to advocate for the needs of African patients and its members currently represent 7 different African countries. Community Outreach - The Health Promotion Department focuses on Brazilians, Latinos, Cambodians, African immigrants and others who do not have health insurance, are not regular users of primary care, or are in need of mental health services. Outreach efforts are coordinated with all LCHC departments including Prenatal Services, Family Practice, Women’s Services, HIV Counseling and Testing, Inpatient and Outpatient Behavioral Health Services, Adult and Pediatric Medical Services, Nutrition and Community Support Programs. In addition, we work in collaboration with multiple community agencies and faith-based organizations to help coordinate referrals and access resources.
The Access to Care Program, funded by the Blue Cross Blue Shield Foundation of MA, hopes to improve the overall well-being of the uninsured population of the Greater Lowell area. The Lowell Access to Care Program is designed to address several needs faced by Lowell's uninsured population, including needs for a source of primary care, insurance coverage, and access to a specialist despite the fact that one is uninsured. The Program will accomplish this utilizing a four pronged approach that focuses on community outreach, insurance enrollment, primary care and specialty care services. The Program works in collaboration with our two local hospitals, the VNA of Greater Lowell, the City Health Department and community based agencies. The program activities will increase the availability of specialty services for the uninsured, community outreach interventions and the improvement of a referral system that will allow for proper tracking of patients, thus providing appropriate linkages to primary care and continuity of care.
The Cross-Cultural Communication Project, funded by the Blue Cross Blue Shield Foundation of MA, hopes to improve the health status of patients through improvements in the patient-provider relationship, specifically looking at cross-cultural communication skills of patients, providers and interpreters. The project targets the communication dynamic between foreign language speaking patient and provider. The project, which builds on LCHC’s CQI Language Access System Improvement Team Project, is three tiered and looks at the delivery of a Promotores(as) program (Spanish and Portuguese for lay health worker), provider training, assessment and development of guidelines for cross-cultural communication skills, and the enhancement of an internal interpreter program that will provide ongoing assessments of language and interpreting skills and interpreter training.
