Lowell Community Health Center is committed to providing quality care for patients of all ages and backgrounds. Our medical staff takes the time to know each patient as a person, to learn the patient’s health history, and to suggest the right treatment plan for that patient.
The Health Center helps patients monitor and manage common, chronic health conditions, such as diabetes and high blood pressure. Many of our health services focus on prevention and education, to help patients take care of themselves and stay healthy.
The Health Center also understands how important it is to coordinate health care among different medical providers. We will help patients find specialists, set up appointments and make sure each medical provider has all the information he or she needs.
The Health Center is open six days a week, including evening hours and Saturdays.
The Health Center has achieved Joint Commission accreditation and Level 3 Patient-Centered Medical Home Recognition for complying with rigorous national performance standards that promote quality health care delivery.
